What you owe after receiving services at a Whitefoord, Inc. Health Centers depends on whether you have health insurance and what your insurance covers. If you do not have health insurance, we can help you understand your options and costs. Learn more about insurance assistance.

No one is denied services at Whitefoord, Inc. due to an inability to pay.

What Insurance Plans are Accepted?

Whitefoord, Inc. provides healthcare for ALL and we accept most health insurance plans, including Medicaid, Medicare, Ambetter, and private insurance plans (like the insurance you may get from your employer).

Plans offered through your employer and plans purchased on the Affordable Care Act Health Insurance Exchange are accepted.

Whitefoord accepts a variety of dental insurance plans and also offers a sliding fee scale, based on your income and family size, if you do not have dental insurance.

2019 Sliding Fee Discount

If you do not have health insurance or need help paying for expenses that health insurance does not cover, we offer a Sliding Fee Discount. The Sliding Fee is determined based on your income level and the number of members in your household.

How do I Qualify for the Sliding Scale?

Whitefoord will help you apply for the Sliding Scale. You may download and complete the application before your appointment and bring it with you. You may also bring the following information to your appointment and complete the form in our offices.

  • A copy of page 1 of your most recent federal income tax return (Form 1040 or 1040A) and a copy of recent wage statements, unemployment or pay stubs a statement/letter from your employer showing year to date earnings can be substituted for pay stubs if one is not available.
  • A copy of your current driver’s license or current utility bill with your name and street address or other legal document is required to prove your identity.

This information is only used for determining your identity and is held in strict confidence.

Once approved, the sliding fee reduction will be good for one year or whenever your income and/or family size changes. If you continue to receive services, you may be asked to re-apply.

What happens if I don’t provide the documentation?

We will compute your sliding fee discount based on your information provided and you will be asked to send in the required information within one week.

Without this documentation, you will receive a bill for the full amount of the charges, which are due within 10 days from the date of service. If you need a return visit to our clinics in the future, we will require that documentation be on file to continue qualifying for the sliding fee discount. If the required documentation is not on file, you will be charged our usual charges for the services provided. Qualifying must be updated on an annual basis or whenever your income and/or family size changes.

Whitefoord is here to help. If you do not have insurance, we can help you understand your option for applying for insurance and other assistance programs available regardless of income or immigration status. We can also help you with your application. One of our eligibility specialists can help you determine if you qualify for:

– Low or no-cost insurance, such as Medicaid
– Insurance through http://www.healthcare.gov (Affordable Care Act also called “Obamacare”)
– Our Sliding Fee Discount Program, where we offer discounted fees for services (see tables above)
– Other assistance programs

If your child attends Atlanta Public Schools, insurance assistance is also available at our school-based health centers at King Middle School, Toomer Elementary School, or at the Whitefoord Early Learning Academy.
