Payment and Sliding Fee Scale
Whitefoord provides access to all our patients regardless of source of payment and accepts most private health insurance, Medicaid and Medicare. We also offer income-based sliding fee scale schedule. Fees on the sliding fee scale are based on income and family size so that quality health care is accessible and affordable. No one is denied services at Whitefoord, Inc. due to an inability to pay.
2025 Sliding Fee Discount
If you do not have health insurance or need help paying for expenses that health insurance does not cover, we offer a Sliding Fee Discount to patients who qualify. The Sliding Fee is determined based on your income level and the number of members in your household. All patients must be recertified annually or at the 1st visit after February 1st of every year.
To apply for the Sliding Fee Scale Program, patients must supply required documentation, including valid photo identification, proof of income for the past 30 days and current address verification. All patients must be recertified annually or at the 1st visit after February 1st every year.
What Should I Bring?
Photo ID
If you have medical insurance, bring a copy of your insurance card.
If you have Medicaid or Medicare, bring a copy of your card with you.
If you do not have insurance coverage, proof of income for the past 30 days is required to qualify for our sliding fee scale program. Re-certification is required annually, or 1st visit after February 1st of every year.
If this is your first visit to Whitefoord, please arrive at least 20 minutes prior to your appointment time.
Whitefoord recently reviewed and updated its Sliding Fee Discount Program patient fees in accordance with the 2025 Federal Poverty Guidelines. What Insurance Plans are Accepted?
Whitefoord, Inc. provides healthcare for ALL and we accept most health insurance plans, including Medicaid, Medicare, Ambetter, and private insurance plans (like the insurance you may get from your employer).
